By completing this form I understand that I am a client of Empire Training and Nutrition.  As a client of Empire I understand that there is a 24 hour cancellation policy for in person sessions.  I will give at least 24 hours notice before canceling an in person session.  If 24 hours notice is not given the session is still paid to my Coach as their time is valuable.  *We understand Emergencies happen and will handle this on a case by case basis. It is the responsibility of the client to check-in with their coach weekly via client weekly update form in order to receive the most benefit out of our services.  If check-ins are not completed we cannot fully execute your plan.  We also require a 30 day advance notice if you need to discontinue our services.
